Incoming Freshmen - 8th Grade Currently Enrolled Catholic School
The Admissions Committee reviews each application individually, considering a combination of factors before making a decision. Please follow the appropriate procedures below to ensure the timely review of your child’s application:
Please Note: All requested documents must be turned in before Entrance Exam day
Incoming 9th Graders from Catholic Schools:
- Entrance Exam form & $50 fee (Exam is on Monday, April 30, 2012 at 8:00 A.M.)
- Admissions Application
- Release of Record Form
- Copy of Birth Certificate
- Social Security Number
- Initial & Most Recent Evaluation (ESE Students Only)
- I.E.P. (ESE Students Only)
- Parish letter, if applicable
- Original Immunization Form* (will be mailed home with registration packet)
- Original Health Form* (will be mailed home with registration packet)
The following documents will be provided to us directly from the Catholic School:
- 7th grade report card
- 8th grade report card
- Standardized test
- School Evaluation Form
