Admissions Checklist

• Step 1. Create an Account

  Admissions Portal Link:https://portal.pacehs.com/login?callbackUrl=%2Fportal%2Fhspt.  

  
  After creating your account, please select:
  

  • • Select "Incoming 9th - Non-Catholic"

• Step 2*. For 9th Grade Applicants Who Have NOT Taken the High School Placement Exam

  Register for HSPT
  

  • • Under "Your Admissions Path" section, please click the "Register for HSPT" button in RED
    • Complete the registration and submit the non-refundable $50 HSPT fee

  Please note: Incomplete registrations will not be able to test on December 5th.

• Step 3. Complete the Online Application

  Please upload the following documents to the corresponding tabs in your portal:

  • • Copy of your child’s birth certificate
    • State of Florida Physical Exam Form DH3040 - Must be current and NOT the athletic physical
    • State of Florida Immunization Form DH680 - NO Religious exemptions will be accepted

  Please note: All required documents must be submitted by November 30th. Registrations with missing documents will be considered incomplete and will not be able to test on December 5th.

• Step 4. Upload Required Documents

  Please upload the following documents to the corresponding tabs in your portal:

  • Most recent 8th-grade report card
  • 7th-grade report card
  • School evaluation – must be completed by your student's English or Math teacher
  • Release of Record
  • Last standardized test scores (e.g., MAPS, PSAT, FSA, etc.)
  • State of Florida Physical DH 3040 - If your student's physical is expired, an updated one must be provided. Please note that this form is valid for only one year.
  • State of Florida Immunization DH680: Must be COMPLETE for 7th grade. NO religious exemptions will be accepted.
  • Pastor Recommendation Form – IF applicable. This form must be submitted directly to the Director of Admissions BY Parish/School Official and NOT uploaded to the portal. If submitted to the portal, it will not be accepted. 
  • Copy of your child’s birth certificate 

  Please note: For your application to be reviewed, all required documents must be submitted. Without these documents, the application process cannot move forward. Incomplete applications will not be considered for enrollment. 

• Additional Documentation (Students with Learning Disabilities)

  Additional Documentation (Students with Learning Disabilities):

  If your child has a documented learning disability, please also provide the following:

  • A copy of the current Individualized Education Plan (IEP) or 504 Plan
  • Initial and most recent psycho-educational evaluation (conducted within the last three years)
  • Details of any accommodations or support services currently provided by your child’s school

  
  

  Please note: If no I.E.P or documentation is provided, your child WILL NOT receive their accommodations for the HSPT.
