Admissions Checklist

Incoming 9th graders from ADOM Catholic Schools:

List of 5 items.

  • Step 1. Create an Account


    After creating your account: 
    1. Select theProgram:Catholic Feeder School 
    2. Choose 9th grade 
    3. SetYear:2026-2027 
    4. Enter Student’s name 
  • Step 2*. For 9th Grade Applicants Who Have NOT Taken the High School Placement Exam

    Register for the HSPT - Friday, January 16, 2026
    • Complete the HSPT registration titled “2026–2027 HSPT Registration.”
    • Submit the $50 HSPT fee
    Please note: Applications missing the fee will be considered incomplete and cannot be reviewed
  • Step 3. Complete the Online Application

    • Visit https://bit.ly/pace2627 to access your student’s application
    • The application will be available through your online portal
    • Application titled "9th Grade Application 2026"
    Please note: Without submissions of the application, the admissions process CANNOT begin.
    1. Step 4. Upload Required Documents

      Please upload the following documents to the corresponding tabs in your portal:
      • Copy of your child’s birth certificate 
      • State of Florida Physical Exam Form DH3040 
      • State of Florida Immunization Form DH680 - NO Religious exemptions will be accepted
      • Pastoral Recommendation formIF applicable. This form must be submitted directly to the Director of Admissions BY Parish/School Official and NOT uploaded to the portal. If submitted to the portal, it will not be accepted.
      Please note: For your application to be reviewed, all required documents must be submitted. Without these documents, the application process cannot move forward. Incomplete applications will not be considered for enrollment 
    2. Additional Documentation (Students w/ Learning Disabilities)

      If your child has a documented learning disability, please also provide the following: 
      • A copy of the currentIndividualized Education Plan (IEP)or504 Plan 
      • Initial and most recentpsycho-educational evaluation(conducted within the last three years) 
      • Details of anyaccommodations or support servicescurrently provided by your child’s school 
      If no I.E.P or documentation is provided, your child WILL NOT recieve their accommodations for the HSPT. 

    Incoming 9th graders from Non-Catholic/Public Schools

    • Step 1. Create an Account

      After creating your account: 
      1. Select theProgram:Non-Catholic/Public 
      2. Choose 9th grade 
      3. SetYear:2026-2027 
      4. Enter Student’s name 
    • Step 2*. For 9th Grade Applicants Who Have NOT Taken the High School Placement Exam

      • Most recent 8th-grade report card
      • 7th-grade report card
      • School evaluation – must be completed by your student's English or Math teacher
      • Release of Record
      • Last standardized test scores (e.g., MAPS, PSAT, FSA, etc.)
      • State of Florida Physical DH 3040 - If your student's physical is expired, an updated one must be provided. Please note that this form is valid for only one year.
      • State of Florida Immunization DH680: Must be COMPLETE for 7th grade. NO religious exemptions will be accepted.
      • Pastor Recommendation Form – IF applicable. This form must be submitted directly to the Director of Admissions BY Parish/School Official and NOT uploaded to the portal. If submitted to the portal, it will not be accepted.

    Transfer Students

    List of 4 items.

    • Step 1. Create an Account


      After creating your account: 
      • Select theProgram:Transfer Student Application 
      • Choose the grade your child will be entering 
      • SetYear:2025-2026 or 2026-2027 
      • Enter Student’s name 
    • Step 2. Complete Application

      • Complete Application titled: “Transfer Student Application” 
      • Submit $75 application fee 
      Please note: If the fee is not submitted, the applicationwill be considered incomplete and cannot be reviewed. 
      1. Step 3. Upload Required Documents

        To proceed with the applicationreview, please upload the following documents to the corresponding tabs in your portal: 
        • Last standardized test scores (ex. MAPS, FAST, FSA) 
        • Unofficial transcripts from your child’s current school 
        • School Evaluation - Must be completed by current English or Math Teacher 
        • Release of Records 
        • One letter of recommendation – Must be on an official letterhead 
        • Copy of your child’s birth certificate 
        • State of Florida Physical Exam (Form DH3040) 
        • State of Florida Immunization Form (DH680)
